Protocol For Icing

When using ice therapy, it is best to wrap a moist cold towel around the ice pack and place it directly on the skin for 10-15 minutes. Moisture assures that the cold will penetrate to the deepest layer of tissue possible. This should be repeated at least three times a day during the acute phase of injury. When icing an extremity (hand, wrist, foot, ankle or elbow), a bucket of half tap water/half ice is preferred. Fill the bucket halfway with ice and then fill with water. Submerge the injured body part for at least 5 minutes, but no longer than 7 minutes. This should be repeated 3-5 times a day during the acute phase of injury.

Phases of Cold Therapy: You should expect to feel the following sensations during cold therapy: 1: COLD 2: BURNING 3: ACHY 4: NUMBNESS. Once numbness has been achieved, the therapy is complete for that session.
Alternating Cold/Hot/Cold: After the acute phase of an injury, it can be beneficial to use alternating cold/heat. For an extremity (hand, wrist, foot, ankle or elbow), a bucket of half ice/half water and a bucket of hot water to tolerance are recommended. Five minutes of cold, ten minutes of hot, five minutes of cold. Repeat three times a day until swelling is down. On areas other than an extremity, alternating moist ice pack/moist hot pack is recommended. Again, 5/10/5, repeating three times a day until swelling is down.
