2009 Individual and Family Plans

| Wellness Within Chiropractic is pleased to provide family plans for any size family. Our Family Share plans are a convenient way to purchase chiropractic care on an annual basis, and may be "shared" between family members as the need arises. A typical family can expect to save approximately 15 - 20% off our normal fees using one of our family plans. |
| Plan Vitality $560 – receive 14 visits at a cost of $40 per visit (saving $5 per visit) – this plan is payable in two installments of $280 |
| Plan Longevity $1140 – receive 30 visits at a cost of $38 per visit (saving $7 per visit) – this plan is payable in two installments of $570 |
Plan Supreme Health $1800 – receive 50 visits at a cost of $36 per visit (saving $9 per visit) – this plan is payable in two installments of $900

| Wellness Corporate Plan $2475 – receive 75 visits at a cost of $33 per visit (saving $12 per visit) – this plan is payable in two installments of $1237.50 – this plan is designed for companies with 3 or more employees |

Notes about Wellness Plans:
· Our first three plans may be used for one individual, partner/spouse or for an entire family.
· Should you unfortunately miss an appointment without giving us 24 hours notice the $25 missed appointment fee can be deducted, should you choose, from your plan.
· Because outstanding bills are stressful for patients, at WWC it is our policy that patients pay at the time of service or are on a prepay status.
